A perch fell down in the cage. The rectangular metal securing piece that goes on the outside had somehow swivelled round and gone through the bars meaning the perch had nothing to secure it. I think Mitzi was on it at the time. I heard a crash and both parrots were on the floor but Mitzi's primary flight feathers were all messed up. I have replaced the rectangular piece with a circular one from a spare perch but most of the perches have the rectangular ones. Mitzi wouldn't go back on it at first although I managed to persuade her with a piece of biscuit.

I think it is annoying that the cage is for Greys and the perches are for Greys but the rectangular metal piece is the same width as the bar spacing. If they had made it a bit wider it couldn't have happened. It was fixed horizontally but had somehow swivelled round and gone through the vertical bars. I think Monty and Mitzi's movement on the perch must have caused it to gradually become loose and swivel round.
